If you plan on making more than $600 per each tax year, we will need to collect your Tax ID Number for Businesses or Social Security Number for Individuals to issue a 1099 at the end of the year. If you make less than $600, we will not issue a 1099 unless directed.

If you plan to make less than $600 and do not wish for a 1099, you may skip this field. If you leave this blank, your payments will be temporary held once you reach the $600 limit. If this occurs, you will be required to supply your Tax ID or Social Security Number to remove the hold.

I hereby authorize TPM or NEXUS to deposit any amounts owed to me, as instructed by my above elections and chosen financial institution. I further authorize to accept any credit entries indicated by TPM or NEXUS to my account. In the event that TPM or NEXUS deposits funds erroneously into my account, I authorize TPM or NEXUS to debit my account for any amount not to exceed the original amount of the erroneous credit with notice.

The contractor agrees that any controversy or claim arising from the employment or services provided to the Company shall be settled by arbitration administered in Chicago, IL pursuant to a single arbitrator arbitration administration by the American Arbitration Association under its Commercial Arbitration Rules and judgement on the award rendered by the arbitrator may be entered in any court having jurisdiction hereof. The contractor agrees that each party will be responsible for their own legal fees. Arbitration will be the sole means of resolving such disputes and both parties waive their rights to resolve disputes by court proceedings or any other means.
